As a duty and responsibiity to others, report all suspicious activities to all related Local, State and Federal Authorities in a timely,
and well substantiated  manner.  Mail your Complaints:  via USPS Registered with return receipt. Secure copies for your file.
The sooner you act  the better it would be for everyone. Early detection and rapid response minimizes damage.

Instructions Difficulty: Easy

Step1
Download IRS Form 911 from the official Internal Revenue Service at IRS.gov.

Step2
Determine if you qualify to use Internal Revenue Service Form 911 by examining the following criteria.

1—You suffer economic harm;
2—You may have an immediate adverse action;
3—You may have to pay more if you don't get relief;
4—You could have troubles that you cannot repair or long-term difficulty;
5—You have a 30 day (or longer) delay in resolving a tax problem;
6—You have not received a response/resolution to your problem by a date promised;
7—A system has not operated as intended or failed to resolve your problem;
8—The tax laws raise considerations of equity or has impaired your rights or
9—Public policy warrants assistance to an individual/group of taxpayers.

Step3
Complete Section I: Taxpayer Information. Provide your name, Social Security number (and spouse's name and SSN, if applicable), street address, city, state, zip code, fax number, email address and employer identification number (EIN). Indicate which tax form you used, either 1040 or 1040EZ, and note the tax period or year for which you are requesting help. Provide your contact information including daytime telephone number, best time to call and special communication needs (such as TTY or an interpreter). Describe the tax problem you're experiencing and the kind of assistance you desire. You and your spouse should sign and date the form.

Step4
Fill out Section II: Representative Information. If you want someone to represent you, first have this person fill out IRS Form 2848 (Power of Attorney). Then provide the following information about your representative: name, Centralized Authorization File (CAF) number, mailing address, daytime phone and fax numbers, and the representative's signature and date.

Step5
File Form 911 with the IRS. First, find the nearest Taxpayer Advocate office in your state by calling (877) 777-4778. Upon locating the office, you may either mail or fax the form directly to that branch. Overseas taxpayers may mail the form directly to Taxpayer Advocate Service, Internal Revenue Service, PO Box 193479, San Juan, Puerto Rico 00919-3479.
